ARC Offers a Team Approach to Long-Term Care

Celebrating 60 years of service to the community, ARC of Greater Milwaukee (ARC) has expanded its programs to better meet the needs of individuals with developmental and related disabilities. On December 1, 2009, ARC began providing Care Management Services in partnership with Milwaukee County’s Care Management Organization as part of the State of Wisconsin Family Care Program. ARC is building its program capacity to provide Care Management Services to 250 individuals.

 

“Providing in-home and community services has been critical to ARC’s mission to improve the quality of life for individuals with disabilities, their families and our community. ARC is pleased to expand into this important service delivery network to develop and monitor service plans that support persons with disabilities as they live, work and contribute to our community,” stated Vicki Spataro Wachniak, ARC Executive Director.

 

Working with the Milwaukee County Care Management Organization, ARC offers a team approach to long-term care. A registered nurse and social worker from ARC, along with Family Care members and their families, work together to coordinate the most effective care possible for each individual served.
